Our Continuum of Care

Our campus provides a continuum of care in one convenient location. We offer assisted living at The Village at Laurel Lake and nursing and rehabilitation at Laurel Lake Center for Health and Rehabilitation. Our campus model allows additional health care should the need arise. Residents, patients and families have the assurance of knowing they have access to the care they require without moving campuses.

At The Village at Laurel Lake , seniors can continue living independently while receiving the extra measure of support they need. Residents have the option of studio, one, or two-bedroom apartments, all with private baths and kitchens. The monthly rental fee includes gourmet meals, housekeeping, transportation, activities and much more. Personal care and medication management are under the supervision of a nurse. The Village at Laurel Lake residents have priority admission status to Laurel Lake Center for Health and Rehabilitation if they require rehabilitative care.

Laurel Lake Center for Health and Rehabilitation adjoins The Village at Laurel Lake. Our transitional nursing and rehabilitation center specializes in short-term, inpatient rehabilitation, bridging the gap between hospital and home for patients needing continued care to recover completely. Patients and residents at Laurel Lake Center for Health and Rehabilitation receive preferred admission status to our assisted living residence, provided they meet the criteria.
